Hi Moonet_Dor!

The problem you are having is because the entire GuildPortal page is a form. You can't have a form within a form. So it might be best to:
  • Save your form as a basic HTML page
  • Upload the file to your GuildPortal file area
  • Save the link to this file
  • Add & customize a "embded page" widget
    • Go to Control Panel > Pages & Content
    • Choose a page
    • Click [ Add Widget ]
    • Enter "embed" in the Search Widgets area, then press enter
    • Click [ Add to Site ]
    • Choose middle, make sure it's going to the right page, then name your widget something like "Contact"
    • Click [ Save New Widget ]
    • When it asks you to customize, click [ Yes ]
    • Now paste in the link from the third step above into the "Embedded Page URL" area
    • Pick a height which will contain all of your page, try something like "200", then you can go back and adjust it as needed.
    • Click [ Save ]
  • Reload your GuildPortal page to see the widget.
All that being said, I'm not sure the HTML you have above will work anyway, I've never really messed around with a mail to form so I couldn't tell you for sure that it'll work. If it doesn't work, you might want to check out Wufoo.
